Parotidectomy

What is Parotidectomy

A Parotidectomy is the surgical removal of all or part of the parotid gland, the largest of the salivary glands, located near the ear. It is commonly performed to treat parotid gland tumors, chronic infections, obstructions, or other abnormalities. At Elegance Superspeciality Dental Clinic, Surat, our experienced oral and maxillofacial surgeons offer advanced care for conditions that require treatment of parotid swelling, parotid fistula, stones, and abscesses.

When Is Parotidectomy Recommended?

This surgery is advised for:

  • Benign or malignant tumors
  • Chronic parotid gland infections
  • Parotid abscess treatment when drainage fails
  • Parotid stone treatment if minimally invasive methods don’t work
  • Treatment of parotid fistula that does not heal conservatively
  • Managing parotid swelling and unexplained enlargement

Parotid Enlargement Causes

  • Salivary stones (sialolithiasis)
  • Chronic sialadenitis
  • Autoimmune diseases (e.g., Sjögren’s syndrome)
  • Viral infections (e.g., mumps)
  • Tumors (benign or malignant)

Parotid Gland Cancer Symptoms

  • A firm, painless lump near the ear or jaw
  • Facial weakness or paralysis
  • Difficulty swallowing
  • Pain or tingling in the region
  • Rapid growth of a previously stable swelling

Benefits of Parotidectomy

  • Removal of potentially cancerous or suspicious masses
  • Relief from chronic pain, swelling, or infections
  • Improved facial aesthetics and comfort
  • Prevention of complications like abscesses, fistulas, or malignant spread
  • Enhanced jaw function, speech, and quality of life
  • Permanent resolution when conservative treatments fail

Risk Factors Involved

Though generally safe, possible complications include:

  • Facial nerve damage (temporary or permanent)
  • Salivary fistula (leakage of saliva from the wound)
  • Infection or hematoma
  • Scarring or cosmetic asymmetry
  • Frey’s Syndrome (sweating while eating)
  • Recurrence of tumors in rare cases

What Happens Before the Procedure

  • Imaging – CT scan, MRI, or ultrasound to assess size and location of lesions
  • Biopsy (FNAC) – Fine needle aspiration to determine tumor type
  • Medical clearance – Health evaluation and medication review
  • Preoperative counseling – Risks, benefits, incision placement, and recovery discussed
  • Fasting instructions – No food/drink 6–8 hours before surgery

What Happens During the Procedure

  • Performed under general anesthesia
  • An incision is made near the ear and jawline
  • The facial nerve is carefully identified and preserved
  • Partial or complete gland removal is performed based on the condition
  • A drain is placed to collect fluid and reduce swelling
  • Wound is closed with fine stitches and covered with a sterile dressing

What Happens After the Procedure

  • You’ll be monitored in a recovery room post-anesthesia
  • Most patients are discharged within 24–48 hours
  • The drain is removed in 1–2 days
  • Mild pain, stiffness, and swelling may last a few days
  • Temporary weakness or numbness of the face may occur, depending on the nerve proximity

Recovery Time

  • Initial healing: 7–10 days
  • Sutures removal: Day 5–7 (if non-absorbable)
  • Full recovery: 2–4 weeks, depending on procedure extent
  • Return to work and normal activity usually within 1–2 weeks (except in total parotidectomy cases)

Post-Operative Care

  • Take prescribed antibiotics and pain medications as directed
  • Apply cold compresses to reduce swelling in the first 24 hours
  • Sleep with head elevated to minimize fluid retention
  • Maintain excellent oral hygiene to avoid infection
  • Avoid strenuous activity, smoking, and spicy foods
  • Follow-up appointments are essential to monitor nerve recovery and healing
  • Facial physiotherapy may be recommended if there is nerve weakness

When to Consult a Doctor

Seek consultation if you notice:

  • A new or persistent lump near the ear or jaw
  • Pain, swelling, or recurrent infections in the parotid region
  • Facial weakness, asymmetry, or difficulty chewing/speaking
  • Pus or saliva discharge from an open wound or skin
  • Rapidly growing swelling or suspected tumor

Early diagnosis and timely surgical care ensure the best functional and cosmetic outcomes — especially in tumor-related or recurrent parotid gland issues.
