Arthrocentesis is a minimally invasive surgical procedure used to treat Temporomandibular Joint (TMJ) disorders. It involves flushing the TMJ with a sterile solution to remove inflammatory byproducts and improve jaw movement. Arthrocentesis is often recommended when conservative treatments such as medications, physical therapy, and splints fail to relieve symptoms like jaw stiffness, pain, and locking.

What is Arthrocentesis?

Arthrocentesis is performed under local anesthesia and involves inserting two small needles into the TMJ. One needle allows the insertion of sterile fluid, while the other allows drainage of debris and inflammatory substances. This lavage helps reduce pressure, remove adhesions, and restore smooth joint function.

When is Arthrocentesis Recommended?

Arthrocentesis is usually recommended for patients experiencing:

  • Persistent TMJ pain or discomfort
  • Jaw locking or limited mouth opening
  • Clicking or popping sounds in the jaw
  • Inflammation of the TMJ due to arthritis or internal derangement
  • Failed response to conservative TMJ treatments

It is especially effective in early-stage internal derangement of the TMJ, where the disc is displaced but not permanently damaged.

  • Imaging tests like MRI or CBCT to evaluate joint condition and disc displacement
  • Review of medical history to check for allergies, medications, or systemic conditions
  • Pre-procedure instructions such as avoiding food 4–6 hours before the procedure (if sedation is planned)

What Happens During the Procedure

  • The procedure is typically done under local anesthesia
  • Two small needles are inserted into the TMJ space:
    • One to inject sterile saline or Ringer’s lactate
    • One to drain fluid, debris, and inflammatory byproducts
  • In some cases, medications like corticosteroids or hyaluronic acid may be injected after lavage
  • Entire process takes 20–30 minutes
  • No incisions or sutures are needed

What Happens After the Procedure

  • Mild discomfort or swelling may occur for 1–2 days
  • Most patients can resume normal activities within 24–48 hours
  • Some may experience immediate relief in jaw movement and pain
  • A follow-up visit may be scheduled within 1 week

Recovery Time

  • Initial recovery: 1–3 days
  • Full recovery and symptom improvement: Within 1–2 weeks

Post-Operative Care

  • Apply cold compresses over the joint for 10–15 minutes, 3–4 times a day
  • Stick to a soft or liquid diet for 2–3 days
  • Avoid yawning, chewing gum, or wide mouth opening
  • Perform gentle jaw exercises as recommended by your doctor
  • Take prescribed painkillers and anti-inflammatory medications as directed
  • Maintain good hydration and oral hygiene
