Hemifacial microsomia (HFM) is a congenital condition where one side of the face is underdeveloped, affecting the jaw, ear, mouth, or cheekbone. It is the second most common facial birth defect after cleft lip/palate. The severity can vary from subtle asymmetry (mild hemifacial microsomia) to more complex deformities involving the skull, spine, or heart.

Early diagnosis and timely hemifacial microsomia treatment are essential for functional and cosmetic improvement. A multidisciplinary team – including oral and maxillofacial surgeons, ENT specialists, orthodontists, and plastic surgeons – typically manages the treatment plan.

Symptoms of Hemifacial Microsomia

  • One side of the face appears smaller or underdeveloped
  • Asymmetry in jaw, chin, cheek, or eye level
  • Microtia (underdeveloped or absent ear)
  • Hearing loss
  • Difficulty in chewing, breathing, or speaking
  • Facial nerve weakness (in severe cases)

Risk Factors

  • The exact cause is often unknown
  • May be linked to vascular issues during fetal development
  • Rarely hereditary
  • Can occur with other conditions, such as Goldenhar syndrome

What is a treatment for Hemifacial Microsomia?

Treatment for Hemifacial Microsomia is highly individualized and depends on the severity of the condition, the child’s age, and the affected anatomical structures. A multidisciplinary team usually includes oral and maxillofacial surgeons, orthodontists, plastic surgeons, ENT specialists, and speech therapists.

What happens Before Hemifacial Microsomia Treatment?

Planning and Evaluation

  1. Clinical Evaluation:
A thorough physical examination is performed to assess facial asymmetry, jaw function, ear deformities, and airway issues.

  2. Imaging & Diagnostics:
CT scans, 3D facial imaging, and audiology tests are used to map bone deficiencies, soft tissue involvement, and hearing impairments.

  3. Growth Assessment:
In growing children, facial growth potential is carefully evaluated to time the intervention for optimal results.

  4. Customized Treatment Planning:
Based on the data, a tailored surgical or non-surgical plan is created. In mild cases, non-invasive correction may be sufficient. In more severe deformities, multi-stage surgical correction is scheduled.

What happens during Hemifacial Microsomia Treatment?

For Mild Hemifacial Microsomia:

  • Orthodontic Treatment: Braces or aligners are used to support jaw and dental development to improve symmetry.

  • Fat Grafting or Dermal Fillers: Autologous fat or synthetic fillers are injected to correct soft tissue asymmetry.

  • Ear Reconstruction: If the ear is underdeveloped (microtia), staged ear reconstruction using rib cartilage or prosthetics is performed.

For Moderate to Severe Hemifacial Microsomia:

  • Distraction Osteogenesis: A small cut is made in the jawbone, and a distractor device is placed. Over weeks, the device gradually lengthens the bone to improve symmetry.

  • Rib Grafts: Costochondral-rib grafts are used to reconstruct the jawbone or temporomandibular joint in growing children.

  • Orthognathic Surgery: In adolescents or adults, corrective jaw surgery is performed to reposition the upper and/or lower jaws for better alignment and appearance.

  • Hearing Aids or Canalplasty: For those with ear canal deformities or hearing loss, assistive devices or ear canal reconstruction may be recommended.

Benefits of Hemifacial Microsomia Treatment

  • Restoration of facial symmetry and aesthetics

  • Improved chewing, speech, and airway function

  • Enhanced hearing (if auditory system involved)

  • Improved psychosocial confidence and quality of life

  • Functional correction that grows with the child (in staged approaches)

Recovery Time

  • Initial recovery period varies from 1–6 weeks depending on the procedure
  • Speech therapy or orthodontic follow-ups may continue for months
  • Long-term monitoring is needed until facial growth is fully complete

Post-treatment Care in Hemifacial Microsomia

  • Regular follow-up with the craniofacial team
  • Maintain oral hygiene, especially after jaw surgery
  • Adhere to dietary recommendations during recovery
  • Provide emotional support to aid psychosocial development

When to Consult a Doctor for Hemifacial Microsomia

Consult a pediatrician or craniofacial specialist if your child shows facial asymmetry, hearing difficulties, or developmental delays in jaw or ear formation. Early evaluation is crucial, especially for mild hemifacial microsomia, which may not be noticeable immediately after birth.

FAQ

It may appear as slight facial asymmetry, often becoming more evident as the child grows.

While complete symmetry may not be possible, surgery and therapy can significantly improve both appearance and function.

Mild cases may be managed during early childhood, while major reconstructive surgeries are often done between ages 6–16 depending on growth and severity.
